Admissions

Services are available to individuals on the New Mexico Developmental Disabilities Waiver program and private pay fee-for-service arrangements may be made. An initial tour is required for all applicants.

Applicants should complete an Application Form [PDF] and submit the form with a copy of his/her most recent ISP, IHP, BSP or equivalent supports assessment and background material to VSA North Fourth Art Center. Application and supporting material can either be mailed or dropped off at 4904 Fourth St. NW, Albuquerque, NM 87107 or faxed to (505) 345-2896. Upon receipt of a completed application, a VSA staff member will schedule a preview day(s) based on the individual’s artistic interests. Following the preview day(s), admission will be determined with input from the individual’s interdisciplinary team.
